This will be possible in Chapter 12 at the ending boss fight against Ramón Salazar. After catching up to the nefarious villain, who hands over Ashley to Major Krauser, Leon shoots the pint-sized terror and sends him to seeming death. Unfortunately, powered by the Las Plagas parasite, Salazar rises with plenty of disgusting tentacles and limbs, ready to douse Leon in acidic goo.

To get that trophy or achievement, players will have to throw a grenade into Salazar's mouth during the fight. This is not the fleshy component that makes up the outer shell of the boss, but rather, the mouth of the part-human part of Salazar that is housed inside the creature. The best time to do so would be when the boss sticks to the ceiling and prepares for a concentrated blast of ooze, this leaves him in a stationary position for a good while.

Take good aim with a grenade, and lob the explosive straight into the head of Salazar, and if done correctly, there will be no priming time as the grenade explodes straightaway, rewarding players with the trophy and achievement.
